Young Adults with Chronic Conditions: Optimizing Treatment and Transition from Pediatric to Adult Care

This course will provide education on state-of-the-art strategies, best practices and take-home tools (transition checklists, sample transition curricula, readiness assessments, transition plans and policies, up-to-date transition coding and reimbursement tip sheet) to optimize:
- Clinical approaches to care of adolescents and young adults with chronic pain, diabetes, cystic fibrosis, IBD, sickle cell disease, HIV, rheumatic diseases, mental illness, substance use disorder, autism, intellectual & physical disabilities, epilepsy, spina bifida, childhood cancer survivors, and solid organ transplant recipients
- The transition of young adult patients with chronic conditions to—and acceptance of these patients into—adult primary and inpatient/subspecialty care: timing/timelines, communication with goal setting, multidisciplinary care integration, checklists, insurance considerations
Optimized Clinical Approaches:
- Strategies to support patients
- State-of-the-art approaches to chronic pain
- What to screen for in a young adult cancer survivor
- Screening for high-risk behaviors/substance abuse and evaluate their impact on chronic illness
- Addressing sexual health/fertility
Transitioning Patients to and Accepting Patients into Adult Care Practices:
- Preventing adverse health outcomes while moving into adult care
- Preparing patients for adult care, helping them with decision-making, self-care and advocacy/self-advocacy with insurance companies and other care providers, independent living, and self-management, teaching skills
- Patient presentation: Navigating College with a Chronic Disease
